Our physician or Medical Director

Your personal physician may continue to care for you and work in collaboration with the hospice teamor you may choose our hospice medical director as your primary physician

Registered Nurses

Provide skilled care and support by understanding and explaining your illness and according your wants and needs. Our hospice nurse is your case manager and will also provide education, options to manage your illness and support your caregivers

CertifiedHospice Aides

Willassist you to make everyday tasks easier (bathing, grooming, dressing) while at the same time, helping you maintain your dignity and independence.

Social workers

Provide support and counseling for you and those close to you by assisting you with other practical concerns, such as alternative living arrangements, financial issues and referrals to other agencies. They also assist you in the preparation of a living will andwith burial arrangements .

Chaplains

Provide non-denominational spiritual support andcounseling for you and your loved ones based upon your spiritual beliefs, practices andneeds. They will listen and offer prayers and supportyou in healing relationships. Our hospice chaplain will also work with your ownclergy and congregation if you desire.

Volunteers

Share companionship with you and your loved ones. They can provide respite for caregivers by sitting with you while your family takes a break . Your volunteer’s specialtalents are matched to your needs

Bereavement Counselors

Are available to assist your loved ones as theygrieve. Bereavement services are provided bymasters-prepared, licensedcounselors and specially trained volunteers, andinclude individual counseling, support groups, workshops and memorial services.
